To the Student:
One form must be submitted for each degree, diploma or certificate you are requesting.
Request for Graduation forms submitted after the advisement period must have the approval of the VP of Instruction.

Additionally, once you have completed all of your requests for graduation, please complete the Graduate Exit Survey and the Job Placement Status form.
These are to be completed only once for the quarter you are graduating, regardless of the number of awards recieved.
